Partnership Registration

Sign Up For Community Partnership

  • In which county will service be provided
  • This is your federal employer id number or your 9 digit ( ##-#######) tax number assigned by the IRS.
  • Please state the “Mission” of your organization and who is your target group(s) in the communities you serve
  • Please describe your method of distributing the product
  • Describe the steps of “accountability” you will have in place to insure the product is received by the intended clients
  • Please list the location of your distribution site, service days and times of distribution (if you make the deliveries to the clients home, please indicate that)
  • Terms and Conditions

    1. To provide Compassion Coalition with written assurance that it is a charitable, tax-exempt, non- profit organization under the IRS provisions of 501(c)(3), or that it is sponsored by such an organization, and to complete organization information requests on forms provided.
    2. To use all foods and products received from the Compassion Coalition warehouse only in a use related to its non-profit status and solely for feeding the ill, the needy or infants without regard to race, color, religion, sex, age, national origin, handicap, marital status or sexual preference.
    3. To neither offer for sale, sell, transfer nor barter the items supplied by Compassion Coalition in exchange for money, other properties or services, or otherwise allow said items to re-enter commercial channels. Furthermore, the agency will not charge a fee of any kind for food or other items received from Compassion Coalition. If a food recipient wishes to make a donation, it can- not be done in conjunction with, nor have any relation to, the receipt of food. Product may not be transferred to another agency.
    4. To accept all items in “as is” condition.
    5. To notify Compassion Coalition of any change in:
    • Internal Revenue Service 501(c)(3) nonprofit, tax exempt status or be designated by a 501(c)(3) agency as its agent.
    • Liability Insurance. (Please provide an updated policy yearly to CCI)
    • Executive Director, contact individuals, address, phone number, services provided or other relevant information.
    6. Agree to immediately contact Compassion Coalition in case of damage, loss or theft of product.
    7. May not require the attendance at religious services, prayer, or “counseling” as a prerequisite or in conjunction with food distribution or receiving food.
    8. To make available to Compassion Coalition for inspection, records indicating the use of food and products received and the number of participants served with items received from the Compassion Coalition warehouse upon request by Compassion Coalition. Participating organizations also agree to comply to the submitting of required reports on the number of individuals served monthly by the 7th of the following service report date.
    9. Maintain record keeping systems to track the use of Compassion Coalition product and the number of clients served and distribution records on file for (4) years.
    10. To provide Compassion Coalition with a specified handling charge per pound as determined by Compassion Coalition upon receipt of monthly billing, to cover the administrative, operating, storage and handling costs of Compassion Coalition on behalf of the participating organization.
    11. Distribute all products received from Compassion Coalition for use within the agency’s designated area, unless special arrangements have been made.
    12. Pantries Only – Accept referrals that are consistent with the agency’s guidelines, assigned geographic or zip code service areas.
    13. Adhere to the rules, regulations and emergency food assistance standards of Compassion Coalition and any government laws that may be applicable to the agency.
    14. To allow authorized representatives of Compassion Coalition to visit the operations of the Participating Organizations that use items received from the Compassion Coalition warehouse at a time determined by Compassion Coalition for purposes of audit and monitoring the site.
    15. You will be billed monthly and you are expected to pay your bill on or before the due date. If the previous month’s bill is not paid, your agency cannot pickup the following month until the previous month’s bill is paid in full.
    Any false statements or failure to comply with requirements of this agreement in connection with Compassion Coalition participation may result in the agency’s suspension and/or termination of services by Compassion Coalition.
  • THIS IS NOT THE SAME AS THE ST-119.1 NYS Tax Exempt Certificate. The document will be a letter from the IRS containing your Employer Identification Number (EIN). Number format is ##-#######

By partnering with us, we can reach more people, impact more communities and help corporate donors get their products where they need to go.

We will work locally, regionally, nationally and globally as needed to accomplish our mission with partners in the public, private, and faith-based organizational communities.
